Tabagismo passivo no Brasil: resultados da Pesquisa Especial Do Tabagismo, 2008

RESUMO: O objetivo foi descrever prevalência e características sócio-demográficas associadas ao tabagismo passivo no domicílio e no trabalho, em participantes (15+ anos) de amostra populacional da Pesquisa Especial do Tabagismo, sub-amostra da PNAD 2008. O tabagismo passivo é definido como a exposição ao tabaco por não-fumante, em casa, no trabalho ou em outros locais fechados, excluídas ocupações ao ar livre. Associações com características sócio-demográficas foram estimadas por análise de regressão logística. Nos 25.005 não-fumantes, a exposição domiciliar é diária para 12,5% e ocasional para 21%. À regressão multinomial (referência: não-expostos), a exposição diária diminui com aumento de idade e tanto a exposição diária como a ocasional diminuem com maiores escolaridade e renda. Comparada à Região Sudeste, há menor exposição diária no Norte e no Centro-Oeste, e maior ocasional no Nordeste. Dos 10.933 trabalhadores, 55% dos homens e 45% das mulheres relataram exposição no trabalho e 67% exposição domiciliar adicional. A exposição no trabalho é maior em homens, nos mais velhos (55+anos) e com menor escolaridade e renda; e menor no Sul. A desigualdade social no Brasil se revela também na maior exposição ao tabagismo passivo daqueles com menor escolaridade e renda.

Tabagismo no Brasil: desigualdades regionais e prevalência segundo características ocupacionais

RESUMO: O estudo descreveu a prevalência do tabagismo diário segundo sexo, idade, renda domiciliar e ocupação dos moradores de 15 anos ou mais, no Brasil e regiões, baseado nos dados da Pesquisa Nacional por Amostra Domiciliar 2008 (PNAD/IBGE). A análise considerou o desenho da amostra e incluiu 252.768 indivíduos. A prevalência de fumo diário no Brasil foi 15,1%, variando de 12,8% na região Norte a 17,4% na região Sul, sendo 62% maior nos homens que mulheres. A prevalência de fumo foi inversamente proporcional à renda domiciliar, sendo 18,6% entre os 20% mais pobres e 11,5% entre os 20% mais ricos. As mesmas tendências para sexo, idade e renda foram observadas nas diferentes regiões do país. O consumo diário de cigarros foi 3% maior entre os trabalhadores comparados com não trabalhadores. Trabalhadores não manuais apresentaram prevalências de fumo abaixo de 10%, enquanto trabalhadores manuais relataram frequências acima de 20%. A associação entre tabagismo e ocupação permaneceu após ajuste para sexo, idade e renda. As desigualdades encontradas devem ser consideradas no planejamento e direcionamento de ações efetivas para redução do tabagismo. Os grupos ocupacionais mais expostos deveriam ter prioridade nas intervenções.

Systematic Review: Impact of Health Information Technology on Quality, Ef?ciency, and Costs of Medical Care

RESUMO: BACKGROUND: Experts consider health information technology key to improving efficiency and quality of health care. PURPOSE: To systematically review evidence on the effect of health information technology on quality, efficiency, and costs of health care. DATA SOURCES: The authors systematically searched the English-language literature indexed in MEDLINE (1995 to January 2004), the Cochrane Central Register of Controlled Trials, the Cochrane Database of Abstracts of Reviews of Effects, and the Periodical Abstracts Database. We also added studies identified by experts up to April 2005. STUDY SELECTION: Descriptive and comparative studies and systematic reviews of health information technology. DATA EXTRACTION: Two reviewers independently extracted information on system capabilities, design, effects on quality, system acquisition, implementation context, and costs. DATA SYNTHESIS: 257 studies met the inclusion criteria. Most studies addressed decision support systems or electronic health records. Approximately 25% of the studies were from 4 academic institutions that implemented internally developed systems; only 9 studies evaluated multifunctional, commercially developed systems. Three major benefits on quality were demonstrated: increased adherence to guideline-based care, enhanced surveillance and monitoring, and decreased medication errors. The primary domain of improvement was preventive health. The major efficiency benefit shown was decreased utilization of care. Data on another efficiency measure, time utilization, were mixed. Empirical cost data were limited. LIMITATIONS: Available quantitative research was limited and was done by a small number of institutions. Systems were heterogeneous and sometimes incompletely described. Available financial and contextual data were limited. CONCLUSIONS: Four benchmark institutions have demonstrated the efficacy of health information technologies in improving quality and efficiency. Whether and how other institutions can achieve similar benefits, and at what costs, are unclear.

Statistical analysis of small-area data based on independence, spatial, non-hierarchical, and hierarchical models

RESUMO: Data associated with spatially contiguous small areas may be modeled via regression on covariates, with error terms that are either independent or are spatially dependent according to which areas are neighbors of each other. But the data may have extra components of variability due to measurement error, which a careful statistical analysis should filter out. The combination of these possibilities leads to four models, three of which are special cases of the fourth: the spatial hierarchical model. A number of new results are developed for the analysis of small-area data: estimation of the measurement-error variance; diagnostics to determine which model fits and predicts better; and a sensitivity analysis to compare an empirical-Bayesian analysis to a Bayesian analysis. A small-area dataset of doctors' prescription amounts per consultation is fitted to all four types of models and used to illustrate the spatial methodology.

State of the USA Health Indicators: Letter Report

RESUMO: Researchers, policymakers, sociologists and doctors have long asked how to best measure the health of a nation, yet the challenge persists. The nonprofit State of the USA, Inc. (SUSA) is taking on this challenge, demonstrating how to measure the health of the United States. The organization is developing a new website intended to provide reliable and objective facts about the U.S. in a number of key areas, including health, and to provide an interactive tool with which individuals can track the progress made in each of these areas. In 2008, SUSA asked the Institute of Medicine's Committee on the State of the USA Health Indicators to provide guidance on 20 key indicators to be used on the organization's website that would be valuable in assessing health. Each indicator was required to demonstrate: a clear importance to health or health care, the availability of reliable, high quality data to measure change in the indicators over time, the potential to be measured with federally collected data, and the capability to be broken down by geography, populations subgroups including race and ethnicity, and socioeconomic status. Taken together, the selected indicators reflect the overall health of the nation and the efficiency and efficacy of U.S. health systems. Specialist human resources for health in Europe: are we ready?

RESUMO: The European Commission's 2008 green paper and 2009 report,1,2 as well as recent reports by the WHO3 and OECD4 signal the increasing international community's interest in human resources for health. Planning of the workforce has emerged as a critical issue in this field. In the open consultation on the Green Paper, support to health systems on workforce planning was welcomed by 80% of the replies.

RESUMO: This is the eighth edition of Society at a Glance, the biennial OECD overview of social indicators. This report addresses the growing demand for quantitative evidence on social well-being and its trends. It updates some indicators included in the previous editions published since 2001 and introduces several new ones, with 25 indicators in total. It includes data for the 35 OECD member countries and where available data for key partners (Brazil, China, India, Indonesia, Russia and South Africa); other G20 countries (Argentina and Saudi Arabia) are also included. The report features a special chapter on the NEET challenge and what can be done for jobless and disengaged youth. It also provides a guide to help readers in understanding the structure of OECD social indicators. All indicators are available as a web book and an e-book on OECD iLibrary.

Slovenia: Health System Review

RESUMO: This analysis of the Slovene health system reviews recent developments in organization and governance, health financing, health care provision, health reforms and health system performance. The health of the population has improved over the last few decades. While life expectancy for both men and women is similar to EU averages, morbidity and mortality data show persistent disparities between regions, and mortality from external causes is particularly high. Satisfaction with health care delivery is high, but recently waiting times for some outpatient specialist services have increased. Greater focus on preventive measures is also needed as well as better care coordination, particularly for those with chronic conditions. Despite having relatively high levels of co-payments for many services covered by the universal compulsory health insurance system, these expenses are counterbalanced by voluntary health insurance, which covers 95% of the population liable for co-payments. However, Slovenia is somewhat unique among social health insurance countries in that it relies almost exclusively on payroll contributions to fund its compulsory health insurance system. This makes health sector revenues very susceptible to economic and labour market fluctuations. A future challenge will be to diversify the resource base for health system funding and thus bolster sustainability in the longer term, while preserving service delivery and quality of care. Given changing demographics and morbidity patterns, further challenges include restructuring the funding and provision of long-term care and enhancing health system efficiency through reform of purchasing and provider-payment systems.

Situação no mercado de trabalho e utilização de serviços de saúde no Brasil

RESUMO: Este estudo compara a utilização de serviços de saúde segundo a situação no mercado de trabalho em três anos e investiga se estão associados após ajuste por características sócio-demográficas e presença de doença crônica. Foram estudados homens de 15 a 64 anos, incluídos na PNAD de 1998, 2003 e 2008 de acordo com a utilização de consulta médica e internação nos últimos 12 meses e visita ao dentista há menos de três anos. A associação entre a situação no mercado de trabalho e o uso de serviços de saúde em 2008 foi estimada pela regressão de Poisson. Foram estudados 33.726 homens. As prevalências de consulta médica e odontológica aumentaram ao longo do período, mas mantiveram-se mais elevadas entre os trabalhadores com proteção social. Desemprego e trabalho sem proteção social foram independente e negativamente associados à menor prevalência de consulta médica e odontológica em 2008. Conhecer as barreiras e aumentar a oferta de serviços com perfil de funcionamento adequado para quem trabalha é fundamental para tentar reduzir as iniquidades observadas. Por outro lado, promover a inserção no trabalho com garantias dos direitos sociais pode contribuir para a melhoria dos indicadores de saúde pública.

Severity of Illness and Ambulatory Care-Sensitive Conditions

RESUMO: This study describes how severity of illness may refine the definition of ambulatory care-sensitive conditions, or ACSCs. Hospital discharge abstract data from Philadelphia were combined with census data to develop population-based adjusted rates of hospitalization for diabetes and asthma, two ACSCs. By stratifying ACSC hospitalization by severity of illness, variations were observed by age, race, and gender. Minority groups may be at higher risk for less access to outpatient primary care and were observed to have higher rates of more severely ill, Stage 3 hospitalization. Geographic map displays indicated wide ranges of age-sex-adjusted rates for high-severity hospitalization in the five-county Philadelphia region. This refined ACSC measure may help to identify specific groups and clinical conditions within a population to assist health care planners estimate health care resources such as facilities, manpower, and programs, as well as to evaluate their outcomes.

RESUMO: O artigo analisa a dimensão e segmentação do mercado supletivo de assistência à saúde no Brasil e o perfil dos seus segurados. Na primeira parte, destaca alguns elementos conceituais necessários à compreensão das características do mercado em saúde. Analisa brevemente a dimensão e o papel do setor privado de saúde em países membros da Organização para Cooperação e Desenvolvimento Econômico (OCDE) e compara com caso brasileiro. Em seguida, realiza um breve histórico do setor supletivo de saúde no Brasil e, por fim, apresenta uma análise do perfil dos segurados, tendo por base os microdados da Pesquisa Nacional de Amostra por Domicílio de 1998 (Pnad/ IBGE).(AU)

Segmentação da demanda dos planos e seguros privados de saúde: uma análise das informações da PNAD/98

RESUMO: Este artigo apresenta uma investigação preliminar da presença da seleção adversa e do risco moral (moral hazard) na demanda ativa por planos de saúde no Brasil, a partir dos dados da PNAD/98. O presente estudo compara indivíduos cujas coberturas decorrem do vínculo de trabalho, com aqueles cujos planos resultam de uma demanda individual às empresas que os comercializam e os que não têm acesso a esquemas assistenciais alternativos ao SUS. A elaboração de uma tipologia de planos de saúde, combinada com variáveis relacionadas com as condições de saúde, utilização de serviços e gastos com saúde, sugere a existência de falhas de mercado. A percepção de uma condição de saúde mais desfavorável parece estar associada à busca de cobertura e o tipo de cobertura com o maior uso de serviços de saúde. Quando analisadas através de dois modelos de regressão logística com múltiplos controles, onde a variável de desfecho é indicadora de seleção adversa ou moral hazard, essas diferenças se atenuam, com exceção dos gastos com saúde. Os resultados não evidenciam uma inquestionável assimetria de informações, mas sinalizam a necessidade de aprofundar o conhecimento sobre as relações entre morbidade, utilização de serviços de saúde, gastos com saúde e tipo de cobertura.(AU)
